What is the purpose of the survey?
The purpose of this survey is to gather information from our community partners and their clients about their current health care needs and priorities. We are trying to reach the Medicaid population in particular. The survey is anonymous and individual survey responses will not be shared.
How long does the survey take?
The survey should take less than 10 minutes to complete. The first 200 participants will receive a $5 e-gift card to Walmart when the survey is completed.
What are you doing with this information?
This information will be compiled and used in the creation of BMRCHP’s Community Health Improvement Plan (CHIP) to address the highest-priority needs in Walla Walla and Columbia Counties, with a heavy focus on Social Determinants of Health.
Will this information be shared?
The overall results will be shared with members of the partnership, local providers, and other agencies that can help address the needs identified through the survey.
Additional Notes
- One survey per family please; if families say they have already completed through another event or agency, thank them for participating.
- Target 18 years old and older; any income/insurance level can participate.
